It is a private space for collaboration between interdisciplinary teams providing education and implementing the Serious Illness Care Program at their institutions. Evaluation and delivery form for the SIC translation files Ingrid See, RN, BSN, MEd, of Vancouver Home Hospice in Canada, led a team who rigorously and thoughtfully translated the Serious Illness Conversation Guide (SICG) and the Family Communication Guide into 11 languages: Arabic, Chinese (simplified), Chinese (traditional), Farsi, Japanese, Korean, Punjabi, Russian, Spanish, and Vietnamese.
