Culturally adapted Advance Care Planning information sessions for Chinese and South Asian communities

Is your community organization interested in delivering Advance Care Planning sessions for people with a South Asian or Chinese background?

By delivering the sessions, more people in your community will have access to credible Advance Care Planning information and resources.

We’ll provide training and a full toolkit of everything you’ll need to host the sessions.

We’ll train two to four of your people (staff or volunteers) in how to facilitate the information sessions. Then we encourage you to host one or more sessions for your community to learn more about Advance Care Planning. You’ll be fully supported through the entire process with ongoing support in planning, hosting and evaluating the sessions.

A bit about the project

In this initiative we are adapting and translating our community-led Advance Care Planning education

model to be more culturally appropriate for people with a South Asian or Chinese background, and

translating it into Punjabi, Cantonese and Mandarin for those who would prefer one of those

languages.

This project has been made possible thanks to funding from Health Canada. The views expressed herein do not necessarily represent the views of Health Canada.
