Approaches to Advance Care Planning and Goals of Care Conversations

$40Hosted by Hospice Palliative Care Ontario and Canadian Hospice Palliative Care Association.
This session will describe an approach to Advance Care Planning and Goals of Care Conversations. The presentation will be followed by a Q&A session. Participants will gain awareness of Person-Centered Decision-Making (PCDM) components, be able to describe how values shape decision-making, and review resources available on having person centered conversations in the context of COVID-19.
