The Pears and Pitfalls of Person-Centred Outcome Measures

$40Hospice Palliative Care Ontario and Canadian Hospice Palliative Care Association invites you to attend a webinar on the Pears and Pitfalls of Person-Centred Outcome Measures.
This session will introduce participants to the use of person-centred outcome measures to drive results in palliative care and Compassionate Community Initiatives.
Learning Objectives
• To define and describe quality of life assessment and person-centred outcome measures
• To understand the value of using person-centred outcome measures to trigger change for individuals, and guide evaluation and improvement in programs and organizations
• To learn about 2-3 different sample tools, how they are used, and how information generated from the tools is captured, collated, and meaningfully used in real practice
• To learn about the Neighbours Survey measure being used in Compassionate Community initiatives
