Webinar: Adapting the Serious Illness Conversation Guide for Pediatrics and Polypharmacy

Many members of the Community of Practice have expressed interest in adapting the Serious Illness Conversation Guide to better serve their specific patient population or practice setting. This month, we welcome Drs. Danielle DeCourcey and Kristie Weir to describe why they chose to use the conversation guide and how they adapted it for use in two different, and very important, care settings.
Dr. DeCourcey will share her adaptation process of the Serious Illness Conversation Guide for use with adolescents and young adults, and a different guide for parents of adolescents and young adults with serious illness. Dr. Weir will discuss her adaptation of the Guide to determine use of multiple medicines in patients. This webinar will have recorded presentations from both speakers but feature a live Question and Answer session.
