Partnership opportunity: Advance Care Planning for people with dementia grant

We’re looking for community organizations to partner with us to implement and evaluate Advance Care Planning activities tailored for people in the early stages of dementia, as well as their family and friends caregivers.

This phase of the implementation and evaluation partnership offers a $2000 grant to cover expenses and will run from March to June 2021.

From us, you’ll receive:

• training for Advance Care Planning facilitators

• access to a comprehensive toolkit of resources for community organizers, trained facilitators, and public participants

• coaching

• evaluation of the Advance Care Planning activities

 

As a community organization partner, we ask that you:

• ensure that 2-4 staff or volunteers attend the training

• deliver two (2) guided group conversation events and two (2) information sessions using the BCCPC curriculum and toolkits

• participate and support the evaluation of the Advance Care Planning activities

We welcome any interested community organizations from all regions across BC.
